ok. Problem solved:
yum install kernel-devel
Now i have make this:
[root@localhost mnt]# ./mlnx_add_kernel_support.sh -m /mnt --make-iso -t /tmp
Note: This program will create MLNX_OFED_LINUX ISO for rhel6.5 under /tmp directory.
All Mellanox, OEM, OFED, or Distribution IB packages will be removed.

Windows Server 2008 R2 works perfect with the driver! And when i install CentOS 6.5 and configure the network and save the error come: eth5 is failed! And this is correct!!! But when i install the mellanox driver with the iso file from add_kernel_support i become the issue with all ports!
All the Traffic from the Ports goes on Cisco Switch Bay 1! eth0,eth1,eth2,eth3,eth4,eth5
When i power off Cisco Switch in Bay 2 and the 10 Gbit Switch on Bay 3 i can ping all ports (eth0,eth1,eth2,eth3,eth4,eth5 and i become a reponse! This is not correct!!!
I need really help.....
